NOTES FOR SUBMITTING EMAIL REQUESTS:

For Rent Assistance:

1) Indicate your full name, address, and phone number. 

2) Identify the dollar amount being requested.
 

3) Attach a PDF or picture (.jpg, .bmp, etc.) file of the page(s) from your Rental Agreement that shows the Landlord's name and mailing address.  Any rent payment we send would need to be mailed directly to the Landlord.

For Utility Assistance:

1) Indicate your full name, address, and phone number.

2) Please indicate the dollar amount being requested.
 

3) Attach a PDF or picture (.jpg, .bmp, etc.) file of the Utility bill that indicates a delinquent balance or Shut-off notice.
